Prenatal Case Management

The prenatal case management program seeks to encourage healthy behavior for mothers in order to deliver healthy babies. A nurse case manager will see the mother monthly throughout her pregnancy and provide two follow-up visits after her baby is delivered.

Prenatal Case Management provides the following services:

  • Pregnancy TestingPREGNANT WOMAN
  • Temporary Medicaid
  • Information & Referrals
  • Education on a variety of subjects:

    • Harmful effects of alcohol, drugs and tobacco to both mother and infant
    • Gestational diabetes
    • Prenatal care
    • Delivery
    • Nutrition
    • Healthy lifestyles                                                                                                    
    • Breastfeeding                                                                                                      
    • Hypertension                                                                                                     
    • Parenting/Basic newborn care
  • After your doctor has heard your baby's heartbeat we are able to let you listen to your baby's heartbeat at each case management appointment.

How Do You Qualify For Case Management?

  • Have a laboratory confirmed pregnancy test - TCHD does pregnancy tests by appointment at both locations
  • Be a resident of Taney County
  • Be eligible for Medicaid
  • Have at least one risk factor that include but aren't limited to the following:

    • Mother's age 17 years or less at the time of conception
    • Mother's age 35 years or greater at time of conception
    • Currently smoking
    • Pre-pregnancy weight less than 100 pounds
    • Previous infant death
    • Multiple fetuses in current pregnancy
    • Prior low birth weight baby
    • Living alone or single parent living alone
    • Considered relinquishment of infant
    • Homelessness
    • Alcohol abuse by client or partner
    • Drug dependence or misuse by client or partner
    • Physical or emotional abuse/neglect of client
    • Physical abuse or neglect of children in the home
    • Partner with history of violence
